  1. New patients bring:
    • Insurance card
    • Current medical conditions and medications
    • Driver’s license or other form of I.D.
    • Previous x-rays less that 5 yrs old (if available)
    • Download new patient forms for adult, child, and privacy.
  2. Digital x-rays, intraoral camera images, smile photos
  3. Meet the doctor: review health history and discuss concerns
